A Follow Study of Bladder Cancer



Abstract
Between 1974 and 1977 50 patients with transitional cell carcinoma were followed for survival, duration and recurrence.
In each case posttreatment follow ups were made either by communication or by direct contact and among the above 50 patients, 40 patients were analysed in detail for a duration of 12 months or longer, up to 4 years.
The results are as follows:
1. The predominant age was 51-70 years and 12.5% of all patients were in the 5th decade.
2. On cell differentiation grades ff or l[ were in the majority and grade j was not evident. Over all grades 1B or IV were recognized to be more frequent than gradee l or II.
3. Stage B was first in frequency and next stage D with 35% of all patients. Stage 0 was not recognized and only 7.5% were in stage A.
4. Treatment in the majority was combined therapy with surgery and radiation. Surgery only was used in 12.5% and radiation only in 15%. For all stages the major surgical method was electroresection or partial cystectomy. Bricker¢¥s operation was done on 8 patients with stage B, which in 1 patient was recategorized as stage D postoperatively.
5. Posttreatment survival showed 8/18 cases of overall mortality in 1 year, 5/10 cases in 2 years and 3/7 cases in 3 years demonstrating no significant differences, although 1/7 cases in 4 years showed an increasing survival and a good followup tendency.
6. This result showed no significant survival differences between ages 40, 50 and 60, but age 70 had a markedly decreased survival with any treatment.
7. For the patients with stage A or B the survival and its duration were similar with all various treatment methods even with Bricker¢¥s operation. In this result the difference could not easily explained between the surgery only treatment group due to small number of cases, but the radiation only group had the least survival duration.
8. This data demonstrated the frequent recurrence of tumor at any time up to 4 years even in stage B. Stage D group showed 5/7 cases of recurrence within 1 year and the highest mortality within 2 years.
This result implies the need of a wide ranged followup study for the better understanding of bladder tumor and selection of the treatment method.
